If you would like your child to be vaccinated for seasonal influenza at our in-school clinic:
- Review consent and VIS (Vaccine information Statement) form in it’s entirety
- More information can be found at CDC.gov
- Fill out paperwork COMPLETELY. All questions must be answered for us to vaccinate
your child. - Return paperwork by school September 26th.
- DO NOT return the form if you do not wish for your child to receive the flu vaccine.
- Your child will have an animal stamp on their hand to communicate back to you that we
were able to vaccinate them.
