Athletic/activity requirements at CHS

Physical Exams

The Idaho High School Activities Association requires that an athlete receive a physical examination and have on record with the school the interim questionnaire prior to his/her first practice in any IHSAA sponsored sport (grades 9-12).  Physicals are required prior to the first day of practice in the 9th and 11th grades and may not be taken before May 1 of the 8th grade year.  The IHSAA provides a copy of the Idaho Health Examination & Consent Form for the schools.  All physicals are valid for two years.  A student will not be required to have an additional physical examination during the 10th and 12th grades unless:

  1. The physician recommends the student athlete have an additional

examination.

     2. The parents request an examination in the Interim Questionnaire.

  1. Affirmative answers on 1-9 of the Interim Questionnaire indicate a

repeat physical examination.

  1. An athlete has transferred to Idaho from another state

*Caldwell High will not be offering physicals for athletics.  Students/Parents are required to make their own appointments with physicians to obtain physicals.

*You may download physical forms and interim questionnaire forms at www.idhsaa.org or you may pick forms up from Caldwell High School.

Individual Eligibility

     Academic — To be academically eligible for athletics, a student must be enrolled full-time and have received passing grades and earn credits in at least five (5) full-credit subjects, or the equivalency, in the previous semester or grading period for which credit is granted.  All Freshman registering in the Fall are eligible.  However, beginning with the second semester, the same academic standards listed above will apply.

Attendance

     In order to participate in athletic practices or games a student must be in attendance the entire day of the practice/game.  Exceptions to this rule are doctor verification for appointments or parent verification for family emergencies.

Caldwell High School  Activity Cards  - $35

All students involved in athletics, music, speech arts, drill team, and cheerleading are required to have an activity card.  No student will be allowed to participate without an activity card.

What Does This Buy?

  1. Free admission to all regular season athletic contests.
  2. Reduced admission to all away athletic contests.
  3. Reduced admission to CHS dances.
  4. Satisfaction of supporting the activities of CHS.

Where Does The Money Go?

 Athletics,   Cheerleading,   Classes of  2011, 2012, 2013, 2014,  Music Dept., Dance/Flag Teams, Drama,   Scholarship Fund,    Speech Arts,   Yearbook,   Student Council
